Patient Registration
Thank you for your interest in a travel health visit at Central Michigan
University Health Services. Please contact us to schedule an appointment
as far in advance of your departure date as possible (a minimum of 6-8
weeks prior to your travel date is recommended). We recommend that
you use the
Travel Health Pre-Trip
Planning Checklist to guide your
preparation.
Please complete the
Patient Registration
Forms and forward them to the Travel
Health Clinic, Foust 202, as soon as possible.
The Travel Health nurse will review them and consult with a Health Services
physician to develop a plan tailored to your specific needs. It is
important to complete the forms as accurately and completely as you can.
If you are planning to visit several countries, list them in the order
that you will be visiting them.
Travel Health visits
are limited in number and require significant preparation, so we ask
that you notify us at least 24 hours in advance if you will be unable to
keep your scheduled appointment. This will allow someone else to use the
time slot. To cancel or reschedule, call the appointment desk at
774-5693. Please note that there is a $20 charge for failure to
notify us in advance if you cannot keep your appointment.
Your visit will include a review of:
- your health history
- immunization record
- current health problems
- medications you are taking (including over the counter drugs and supplements)
- precautions specific to your health status
- travel health recommendations for the countries on your itinerary
The charge for a Travel Health visit is $45 and is not usually covered
by health insurance plans.
Charges for immunizations and preventive medications are in addition to
the consult. If a more complex assessment is required, including a
physical exam by a clinician or laboratory testing, there will also be
additional charges. These may be billed to your primary health insurance
plan if they are a covered benefit. Again, preventive health care is not
always covered by insurance. You are advised to check with your plan
representative in advance of your appointment to determine your
coverage.
Payment is not required on the date of service. Charges can be placed on
your CMU account. You are responsible for payment of the total amount by
the posted due date on your monthly statement. If you prefer to pay when
you are seen, you may do so by cash, check, money order, credit card or
bank debit card in our Business Office, Foust 108.
