Location, Hours, Parking, Contact Information

The CMU Health Services Pharmacy is located in Foust 106 on campus, near the corner of Preston and East Campus Drive (across the street from the new Health Professions Building). See map. Use the Preston Street (north side) entrance or the south side entrance for convenient access. The Preston Street entrance has a power entry door.

The Pharmacy is open Monday-Friday, 8:00 a.m.-5:30 p.m., during Fall and Spring Semesters; Monday-Friday, 8:00 a.m.-5:00 p.m. during the Summer Sessions.

Convenient short-term parking is available in designated areas in Lot 29 at the east end of Foust.

Eligible Users

The CMU Health Services Pharmacy fills prescriptions for CMU and Mid Michigan Community College students, CMU faculty and staff, their spouses and dependents age 14 years or older, as well as for other authorized users, e.g., Summer Camps and Conferences participants.

To Fill a Prescription

  • New Pharmacy Users

The first time you bring in a prescription, we will fill it while you wait. At that time, we will ask you to provide certain information about yourself for our records:

    • CMU (or MMCC) picture ID card  for CMU and MMCC students; CMU ID card or driver’s license for CMU faculty, staff, spouses and dependents age 14 years or older
    • Prescription drug plan card or photocopy of it (front and back)
    • Complete a Patient Registration Forms (HS 364 A/B)*

To save you time, you can print the form from our website and complete it in advance. (See www.healthservices.cmich.edu, Downloadable Forms.)

o        Sign a form acknowledging that we have offered you a copy of our Notice of Privacy Practices as required by the HIPAA Privacy Rule.

o        Sign an authorization to permit us to bill your prescription drug plan.

  • New Prescriptions

New prescriptions can be dropped off at the pharmacy (Foust 106) or called (989-774-6590) or faxed (989-774-6665) directly from the physician’s office to the pharmacy.

  • To Transfer A Prescription

Bring your prescription to the Pharmacy or call the pharmacy at (989) 774-6590 with the following information:

    • Name of the pharmacy where the prescription was originally filled
    • Telephone number of the pharmacy where the prescription was filled
    • Name, dose, frequency of the drug
    • Prescription number (7 digit number) on the bottle or package

The CMU Pharmacy staff will contact the outside pharmacy and arrange the transfer of the prescription. Please allow 1-2 business days for the request to be processed.

  • 90 Day Refills for CMU Express Script Subscribers

For maintenance medications, e.g., blood pressure drugs, new prescriptions will be filled for only one month the first time the medication is dispensed. This is intended to avoid having medication that can’t be returned or used in the event of an adverse reaction or lack of effectiveness during the first few weeks on the medicine. After the first month, Express Script subscribers can obtain 90 day refills from the pharmacy with an order from their physician.

  • Refilling Prescriptions

There are 2 ways to request a refill of a prescription you have had filled at our CMU Pharmacy:

 1.  Convenient  twenty-four hour online ordering at http://www.rxrefill.cmich.edu, or

 2.  Call the Pharmacy at (989) 774-6590 to request a refill.

In both cases, you should have your prescription bottle handy with the name of the medication and seven digit prescription number on the label.

On-Campus Delivery for Faculty and Staff

If you would like to have your prescription delivered to you on campus, you will need to complete a brief delivery request form and designate who may sign for the delivery in your absence. The form is available at the pharmacy and at http://www.healthservices.cmich.edu, Downloadable Forms.

If your prescription request is submitted to the pharmacy by 10:00 a.m., every effort will be made to deliver it your campus location that same afternoon. Until we have developed a full inventory of medications for faculty and staff, we may not have every drug on hand initially. However, we will usually have it within 24 hours and will deliver it to you the next day whenever possible. After that, we will keep the medication in stock for future refills.

The prescriptions are delivered in bags that give no indication of the contents. They can only be delivered to the patient or the authorized delegate as indicated on the delivery form.

Pharmacy services are confidential and information will not be released to anyone without the patient’s knowledge and signed authorization, except as required or permitted by law.


Over The Counter (OTC) Items

The CMU Health Services Pharmacy carries a number of over the counter items for sale to CMU and MMCC students, CMU faculty, staff, their spouses and dependents age 14 years or older. The items are available at the Pharmacy, Foust 106, Monday-Friday, 8:00 a.m.-5:00 p.m. for walk-in service. Other items are available by request.

A link to an OTC Order Form is provided here for your convenience. You can fill out the form and give it to the pharmacist to fill, call the order to the pharmacy at 774-6590 or fax it to the pharmacy at 774-6665. Phoned or faxed orders can be picked up at the pharmacy, Foust 106 the same day.


Pharmacy Payment Options


  • Many prescription drug plan cards are accepted at the CMU Health Services Pharmacy. You remain responsible for payment of any co-pays or deductibles.

  • You may pay by cash, check, money order, American Express, Discover Card, Master Card, Visa, or bank debit card at the time of your visit.

  • You may have the charges posted to your personal CMU account (CMU students, faculty, and staff only). This includes co-pay charges.

  • If you are a faculty or staff member and have prescriptions delivered to you on campus from the CMU Pharmacy or have a spouse or dependent who also has prescriptions filled there, you may sign up for Virtual Terminal service. This allows you to store credit card information on a secure site at University Health Services and authorize credit card payment for pharmacy orders for yourself, your spouse, and dependents age 14 years or older.

    Using Virtual Terminal eliminates the need to physically come to Foust Hall or Receivable Accounting to pay the charges. It also avoids having to track individual statements for each family member. For additional information or to enroll in the service, stop by the UHS Business Office, Foust 108, or call 774-3059.

  • CMU payroll deduction is NOT an option for payment of pharmacy charges.

  • We are working on a way to create "family accounts" for pharmacy charges on SAP so that an employee’s spouse and dependents can be included on the employee’s online and hard copy statement, eliminating individual statements for each family member. We hope to offer that option in the not too distant future.
