The staff of
University Health Services Business Office is ready to assist you
with your questions and concerns related to medical billing, health
insurance, and medical records. The Business Office is located in
Foust 108 and is open Monday-Friday, 8:00 a.m. to 5:00 p.m. You can
stop by or call them at 774-3059.
Privacy of Protected Health Information (back to

All
information in the medical record, including information about
billing is confidential. Except as required or permitted by law, it
will not be released to anyone without the patient’s authorization.

Practices is offered to each patient at their first visit. It is

University
Health Services operates on a fee for service basis. Charges are
competitive with those in the local community and based on the same
strict guidelines and coding systems as other health care organizations.
Office visit
charges vary depending on whether the patient is new or established with
our practice, the complexity of the problem, and the decision making
involved. Pharmacy and laboratory fees are in addition to the office
visit fee. Questions about fees for specific services can be answered by
calling our Business Office at (989) 774-3059.
Payment for Services (back to

Payment is not
required on the date of service. However, patients who wish to do so may
pay at the Health Services Business Office at the time of their visit by
cash, check, money order, MasterCard, Visa, Discover Card or bank debit
card. If payment in full is made on the date of service, no charges are
placed on the patient’s CMU account.
If the patient does not
pay at the time of the visit, there are two options available:
1)
The total charges for the visit can be placed on the patient’s personal
CMU account. The patient is responsible for payment of the total amount
of charges by the posted due date on the monthly statement from CMU
Receivable Accounting. Health Services charges appear as a line item on
the statement as a “Health Services” charge and a total dollar amount.
There is no information on the statement stating the diagnosis or the
specific services provided.
2)
The charges may be billed to the patient’s primary health insurance.

University
Health Services will bill the patient’s primary health insurance
plan if the patient provides current insurance coverage information
(insurance card or photocopy of it) and signs an authorization at
the time of the visit to allow us to do so. An itemized statement,
suitable for submission to a secondary carrier, is available upon
request from the Business Office, Foust 108.
University Health Services participates with Michigan Blue Cross Blue
Shield (BCBS) traditional plans, BCBS PPO (including CMU Community Blue
PPO), MESSA traditional, Medicare, Medicaid straight, Tri Care (US
Military) standard, and the CMU Student Health Insurance plan.
We
do not participate with Blue Preferred Trust or BCBS Informed Choice
Plan. Although we do not participate with the CMU Faculty MESSA
Tri Med Point of Service (POS) plan, it does allow faculty members
to be seen at Health Services for a 10% co-pay and no deductible. To
receive the $5.00 In-Network co-pay, the faculty member would need to
obtain an authorization from their primary care physician to have the
care provided at Health Services. CMU students enrolled in other
MESSA TriMed POS plans may have co-pays and deductibles for going out of
network to be seen at Health Services.
The patient remains responsible for payment of any co-pays or
deductibles required by the health insurance plan.
Managed Care Plans: HMOs and PPOs (back to

Patients
enrolled in an HMO, PPO, or POS plan who wish to have their plan pay for
the care provided at University Health Services must contact their
plan’s service representative or their primary care physician about
authorization for non-emergency out-of-network care, prior to being seen
at University Health Services. If authorization is denied by the
insurance carrier, the patient can still be seen at University Health
Services on a fee for service basis.
CMU Student Health Insurance Plan for 2007-2008 to be offered by the Student Insurance Division of the United HealthCare Insurance Company. Only CMU students, their spouses and dependents are eligible to purchase the plan.

The 2007-2008 Central Michigan University Student Health Insurance
plan is offered by the Student Insurance Division of the United Health Care Insurance Company. The preferred provider
organization (PPO) portion of the plan is PPOM with which most Michigan hospitals participate. Brochures and
